Description | Gastrooesopgageal reflux disease (GORD) is a condition when the outflow of a stomach content brings on discomfort or complications in the area of the digestive tract, swallow ways as well as in the area of the upper respiratory tract. The diagnostics of GORD and its complications treatment belongs to gastroenterology, otorhinolaryngology, pneumology, alergology or stomatology. The prevalence of off-mouth (laryngopharyngeal, extraoesophageal) problems increases in the consequence of the GORD. In our article, we have reviewed diagnostic methods and their validity for diagnostics not only of the gastroenterology symptoms but the extraoesophageal symptoms of the reflux disease as well. The treatise describes and evaluates the common and different features of the reflux disease. It also deals with the methodology of ph-metry and impedancy mesurement in order to improve extraoesophageal reflux disease (EER) diagnostics. It shows the differences in the standard phmetry and impedancy mesurement for the gastroenterology diagnostics as well as impedancy mesurement for the diagnostics of the extraoesophageal symptoms. It presents laryngoscopical illustrations, specific to vocal defects in relation with EER. Organic disorders in the area of the lower oesophagus sphincter as well as function stress mechanisms of disease development represent most important part in etiology of this condition. The therapy of reflux disease must be comprehensive and linked to the most exact diagnostics. The IPP therapy is one of options, however, the surgical treatment in the case of the lower oesophageal sphincter insufficiency should be the rule. Due to great complexity of the etiology GORD is the illness with a large recurrence, about 36-82%. The interdisciplinary approach of gastroenterologist, otorhinolaryngologist, pneumologist, psychiatrist and others is necessary for high-quality diagnostics and treatment. |
