Popis | Localized scleroderma (LS. morphea) is an autoimmune disease. Although it is not a life thieatening disease, it can affect quality of life in a very negative way. Spontaneous remissions are described, but LS activity usually persists many years. Different treatments are used - topical corticoster-oids and vitamin D3 derivates, immunosuppressants as methotrexate in monotherapy or in combination with systemic coi ticosteroids. Phototherapy broadens therapeutic options with minimal adverse effects. PUVA is traditionally used in the treatment of LS in the Czech Republic, but UVA1 and UVB 311 nm are other possibilities, effective and eligible. We describe a case of a 30-year-old female patient. She suffered of localized scleroderma, in inflammatory stage, and was treated with narrow band UVB. 2 months after the UVB treatment was stopped, a flare of the disease occurred and UVA1 therapy was chosen with a very good effect. Erythema regressed and control ol the disease (no new lesions, hyperpigmentation of the old lesions) was achieved. Phototherapy UVA1 was applied three times a week initially, starting with 1,4 J'Cin , increasing the dose by 0,4-0.6 J cnr. lengthening the intervals between the sessions to once a week. Cumulative dose was 93,4 J,cm . with 19 sessions in total. |
