Description | The discussion about fluids and their amounts in septic patients have divided the professional healthcare workers into two almost irreconcilable groups. Aggressive fluid resuscitation of sepsis within the Early Goal Directed Therapy (EGDT) concept has been a cornerstone of the treatment of septic patients for many years. However, more than 11 years after the original work of E. Rivers, there is growing amount of evidence against this concept (ProCESS, ARISE and PROMISE studies, among others). At the same time, works appear denying this concept even in the pathophysiological point of view. Our knowledge of fluid resuscitation, pathophysiology of sepsis, glycocalyx, and fluid responsiveness have undergone fundamental changes that need to be taken into account and we should proceed rather in terms of conservative, hemodynamically oriented fluid resuscitation. In the lecture we will try to present information from relevant studies and research that support this conservative approach to fluid resuscitation. |
