Description Aim: The aim of this study was to describe the spectrum of MR findings in patients with a history of acute leukemia who recently had a positive cytological finding in cerebrospinal fluid. Method: Retrospective analysis of imaging examinations (MRI of the brain and/or spinal cord) in patients with leukemic CNS infiltration confirmed by analysis of the cerebrospinal fluid. Results: In the time period of the last 11 years (2006-2017), eight patients with CNS infiltration by leukemic cells were diagnosed in our hospital. Six of these patients (2 females, 4 males, age range 26 to 71 years) underwent a MR examination of the brain and/or spinal cord (depending on the clinical picture), revealing abnormal findings in 4 cases. Pachymeningeal enhancement (n = 0), leptomeningeal enhancement (n = 2), cranial nerve enhancement (n = 0), parenchymal brain involvement (n = 4), parenchymal spinal cord involvement (n = 1). Conclusion: MRI examination of the spinal cord and/or brain showed an abnormal findings in 4 of 6 patients with CNS leukemia.
