Description Mechanical thrombectomy (MT) of symptomatic cerebral arterial occlusion in the anterior circulation is a standard treatment in acute Ischemic stroke (IS) with the first 6 h of stroke onset. Acute IS due to basilar artery occlusion represents a generally accepted exception, when MT may be performed beyond the 6 h of symptoms onset as a life-threating intervention. Results of two randomized clinical trials published in 2018 showed clinical benefits and efficacy of MT in the treatment of acute symptomatic cerebral arterial occlusion even beyond the standard therapeutic time window in IS patients selected according to the size of ischemic penumbra or ischemic core. Positive results of these two trials, new recommendations of the American Heart Association/American Stroke Association from 2018 and of the European Stroke Organisation and the European Society for Minimally Invasive Neurological Therapy published in March 2019 led to a need of updating valid Czech recommendations for MT of IS from 2016. Presented recommendations are the consensual statement of the committees of the Czech Stroke Society (a part of the Czech Neurological Society CMA JEP) and of the Czech Neurointerventional Society (a part of the Czech Society of Interventional Radiology CMA JEP).
