Description | Neuropathic pain is currently defined as a pain arising as a direct consequence of a lesion or disease affecting the somatosensory system. Successful treatment depends mainly on establishing the correct diagnosis of neuropathic pain. The level of certainty with which the pain in question is neuropathic in an individual patient can be determined by the grading system (possible, probable and definite neuropathic pain). Treatment of neuropathic pain may be difficult and not always successful in some patients. When choosing the most appropriate drug, clinicians should follow the guidelines for the treatment of neuropathic pain, where the individual drugs are classified as the first-, second-, third- or fourth-line drugs according to their clinical effect and risk. Among other drugs, opioids can also be used for the treatment of neuropathic pain, with tramadol being classified as the second-line drug, and strong opioids currently being classified as the third-line drugs. The recommended opioids in the treatment of neuropathic pain are tramadol, oxycodone, tapentadol and morphine. |
