Description This case report describes the extraction of adult canine heartworm in a 5.5-year-old female Pug and about 7 year-old female Kavalier King Charles Spaniel. Both females were imported from the southwestern region of Slovakia. Both were positive for Dirofilaria antigen by SNAP test. The presence of microfilariae was consequently verified by means of PCR. The first patient had symptoms of caval syndrome with dyspnea, right atrial and ventricular dilatation and adults imaging by ultrasound. Fourteen adults of Dirofilaria immitis were extracted by catheterization. In the second case, the female was asymptomatic, and the first attempt at extraction without skiascopic control failed to successfully insert a catheter from the left-sided jugular vein into the unenlarged right ventricle. In a second attempt from the right jugular vein under skia control, the extraction of three adults was successful. The course of the procedure and recovery was uneventful in both cases. We were able to verify the feasibility of extracting adult heartworms in massive symptomatic and mild asymptomatic infestations with reasonable risk and efficacy. The procedure is very effective and relatively safe if uncomplicated
