| Description | Periodontal defects that are not related to the papilla have long been of interest to regenerative periodontology. The so-called gold standard is considered Christian approach. But it is related to a number of complications, the cause of which is the incision and subsequent suture directly above the treated bone defect. "Lateral Approach" is defined by a sulcular incision and a vertical incision on the opposite side of the treated tooth. It thus preserves intact soft tissues above the defect, which increases the chance of regeneration. The objective of the prospective series was to verify the effectiveness of an approach designed specifically for vertical defects related to the edentulous ridge. A total of seven defects distal to the second lower molars were treated, in combination with amelogenins and bone graft. Changes in clinical attachment level (CAL) and probing depth (PPD) were assessed after 6 months. Pocket closure (PPD ? 5 mm) and primary healing was achieved in 100% of cases, with no complications in terms of suture failure or material exposure. Mean CAL improved from 6 to 3 mm, PPD from 6 to 4 mm (p < 0.05). The "Lateral Approach" proved to be a relevant technique in a pilot study. Especially in the case of pockets in the second molars, which are often related to the extraction of the third molars. |
