| Description | The paper presented here deals with possibilities of a form teacher’s work evaluation. To do so, it uses examples of questions and answers from research questionnaires that reflect teachers’, parents’ and pupils’ opinions concerning the educational activity of form teacher. The special attention is given to form teachers’ activities aimed at strengthening pupil’s mental, physical and social health.
