Description | Empirical antifungal therapy is considered to be a standard procedure in high-risk neutropenic patients. Historically, the first antifungals examined in this indication was amphotericin B, with the entry of newer drugs lipid and liposomal amphotericin B, azoles and echinocandins were examined in clinical trials. Despite the uneven design of studies and assessment criteria of effectiveness no substantial differences in the efficacy of individual antifungals were observed, the drugs differed significantly in toxicity. The selection of an appropriate antifungal agent is in the hands of a doctor who is weighing multirange epidemiologiempirická cal, pharmacological and clinical factors for the benefit of every particular patient. In the light of current knowledge and availability of modern diagnostic methods, it is clear that some patients are over-treated when using empirical strategy and therefore it is necessary to confront the strategy of empirical therapy with preemptive approach. |
