Description | It was made 420 liver transplantations in the transplant centre in Brno during the period from February 2, 1983 till December 31, 2011. Uncommon and serious fungal infections were observed in 5 patients of them. There were one case of Cladophialophora bantiana and Fonsecaea compacta infections, and 3 cases of Cryptococcus neoformans infection. Three patients died due to these infections (3/5=60 %). The authors report 44-year-old man with brain and lung abscesses caused by Cladophialophora bantiana. Despite the antifungal and surgical therapy the infection progressed and lead to the death of this patient. In the second patient, 54-year-old woman, relapsing skin infiltrates appeared with histological, microscopic, and culture detection of Fonsecaea compacta. The abscesses were repeatedly removed by surgeon but only antifungal infection lead to their definitive vanishing. This patient is 7 years without the manifestation of fungal infection. The third patient, 56-year-old man, had dissemination of Cryptococcus neoformans infection to the skin by blood. Surgery and long-term antifungal infection lead to the regression of infection a stabilization of the patient’s status. The forth patient, 56-year-old man, had early encephalitis and bilateral bronchopneumonia. He died 40 days after liver transplantation. Cryptococcus neoformans was detected post mortem by microscopic examination. The last patient, 31-year-old man with primary biliary cirrhosis underwent liver transplantation with reconstruction of biliary ducts. The transplantation and early post-transplant period were without any complication. In the day 12 the patient lost consciousness and in the lumbar puncture Cryptococcus neoformans was detected. In spite of antifungal therapy this patient died after 3 month due to brain edema. |
