Description Ticagrelor is an oral, reversible, direct inhibitor of the P2Y12 type adenosine diphosphate (ADP) receptor characterised by a faster onset of action and by a more expressed antiaggregatory activity compared to clopidogrel, and it does not belong to the thienopyridine group. In the major PLATO multicentre study (180 mg loading dose, 90 mg b.i.d.) ticagrelor was tested against clopidogrel (300–600 mg loading dose, 75 mg daily) in 18,624 patients suffering from acute coronary syndrome with and without ST elevation. After 12 months, the composite of CV death, myocardial infarction and cerebrovascular accident had occurred in 9.8% of ticagrelor- -treated patients versus 11.7% of clopidogrel-treated patients (p < 0.001). No difference was observed in the occurrence of severe bleeding (11.6% vs. 11.2%, p = 0.43). In December 2010, ticagrelor was also registered in the Czech Republic, under the Brilique trademark; since June 2012 it has been covered by health insurance and some centres already have initial clinical experience with it.
