Description In 2013, new European recommendations for diagnosis and treatment of hypertension were issued. These recommendations confirmed the classification of hypertension already approved in 2003 (3 degrees of hypertension, 3 degrees of pre-hypertension and isolated systolic hypertension) and they list 5 basic groups of antihypertensives suitable for both initial and long term treatment. Diuretics belong among these groups. Their primary indication in patients with hypertension is higher age coupled with isolated systolic hypertension, along with shortness of breath and swellings. The most commonly prescribed group of diuretics are thiazides and similar preparations, which however tend to be poorly tolerated and they frequently have adverse metabolic effects. Diuretics are primarily used in a combined therapy. The most commonly used diuretic in combined treatment is hydrochrorothiazide, which is combined mostly with blockers of the renin-angio-tensin system. A modern and promising combination is an ACE inhibitor + indapamide. Combining beta-blockers with diuretics is inadvisable.
