Description | The case of 33-year-old man injured right arm from the press. There was a special circular tear and loss of skin, subcutaneous tissue and superficial fascia of the right forearm. They were bruised with both hands and wrists. Primary treatment under general anesthesia consisted of a revision of wounds, fasciotomy muscle compartments on the right forearm, on both hands and wrists. Was introduced continuous peripheral nerve analgesia (supraclavicular block right). Given the extent and location of injury, we did not indicate reconstruction using lobe sculptures. Do defects totaling 2/3 right forearm were applied dermal replacement IntegraR. Over the next eight weeks we carried out autotransplantation neodermis dermoepidermal autologous grafts. After necessary (relatively long) period of immobilization right arm followed by intense rehabilitation in the constitutional treatment, and later on an outpatient basis. Functional and aesthetic results are eighteen months after injury satisfactory. Treatment was our terms costly, but anticipated patient's return to normal life and the work process, these costs will surely exports. |
