Description The spine is one of the most frequent places of metastatic spreading. In the most cases the metastasis is located into vertebral body where it could cause spinal instability, pathological fracture, pain or neurological impairment. For surgery type planning is important to know performance status, oncological diagnosis, neurological finding and level of affected spine. Biopsy is indicated in unsure diagnosis. Acute decompression of neural structures or decompression with stabilisation are indicated in case of worsening neurological impairment. Radical metastasectomy with stabilisation or total vertebrectomy are indicated procedures if patient has a long life expectancy and good performance status. The goals of spondylooncological surgery are spine stability, reduction of pain and neurological impairment and also oncologicaly radical removal of spinal metastasis.
