Description By presenting a case report of a patient with a suspected testicular tumor, we want to inform and raise awareness of the rare possibilities of differential diagnosis of testicular lesions. Introduction Splenogonadal fusion (SGF) is a rare inborn developmental anomaly of unclear etiology known since 1883. The unique tissue connection, predilectionally to the left, occurs prenatally between the 5th and 8th gestational weeks before the gonads descent. It is more common in men (15: 1), in 33% concomitantly with cryptorchidism. The clinical picture follows the acute scrotum syndrome and may imitate a testicular tumor.
