Description | The chapter describes the issue of bulbar symptomatology in neurological diseases with progressive course resulting in a need of palliative care. Particular sections of the chapter are focused on dysphagia, speech disorders, and sialorrhea (excessive salivation, drooling). The dysphagia section describes its diagnostic possibilities including screening, clinical testing and objective (flexible endoscopic evaluation of swallowing - FEES, videofluoroscopic swallowing study- VFSS) and questionnaire examination. Possible compensatory strategies and indications for tube feeding (percutaneous endoscopic gastrostomy - PEG) are presented in the dysphagia therapy section. The section focused on the speech disorders describes the issue of dysarthria, its diagnostic methods and treatment options including alternative and augmentative communication. In the last section, clinical presentation of sialorrhea is described including its pharmacological treatment options. Furthermore, each section of the chapter contains the description of the changes of particular bulbar symptoms and signs during the common course of progressive neurological diseases and related changes of the diagnostic and therapeutic options in particular clinical situations with emphasis on the patients' quality of life. The chapter contains also a case report of progressive bulbar symptoms in a patient with amyotrophic lateral sclerosis. |
