Description | Introduction: Bariatrics as quite young surgical field is gradualy siezing to be taken as „cosmetic“ surgery. Most specialists are now attributing its benefits to improvement of metabolic syndrome rather then weight loss. In our department we prefer malabsorptive methods over those restrictive. The work presents the team experience since 2007 from a university department till recent district hospital department alongside the evolution from gastric banding, sleeve gastrectomy up to gastric bypass with one or two anastomoses. Aim: The goal of the study is to present our department and to show our short term results of Roux-Y gastric bypass with circular anastomosis(RYGBc) compared with one anastomosis gastric bypass (OAGB) and sleeve gastrectomy (LSGE). Methods: All patients who undergone a baratric surgery in Hospital Trebic since 2021 (limited size is attributing to the stop of elective surgery due to covid-19 pandemics). Following parameters were tested: BMI, commorbidities (T2DM, elevated blood pressure, hyperlipidemia), medications regarding those commorbidities and objective signs of their resolution. These parameters were tested before the surgery, then 3,6 a 12 months after the procedure. No patients from previous departments of our team were included.In this work we also present our technique of circular gastrojejunostomy. Results: 45 OAGBs, 20 LSGEs and 5 revisional procedures were performed. In all groups the tendency to reduce medications was visibl with statistically non-significant better results of OAGB or RYGB vs LSGE. Conclusion: Due to significant improvement of commorbidities, which were till recently a domain of conservatieve medical fields, i tis necessary to také metabolic surgery as a early and important part of these commorbidities treatment algorithm (T2CM, elevated blood pressure, hyperlipidemia).The importance of metabolic surgery will rise alongside the epidemics of obesity in Czech Republic. |
