Description Radical prostatectomy in primary metastatic prostate cancer remains controversial. With the availability of sensitive imaging methods and the detection of so-called oligometastatic disease, an increased interest in the topic has arisen in recent years, suggesting a benefit of combination of local and systemic treatment for selected patients. The article provides a summary on the role of cytoreductive radical prostatectomy in the treatment of metastatic prostate cancer with an emphasis on potential indications, perioperative results, multimodal treatment, and oncological efficacy based on currently available, heterogeneous data of varying quality. In the absence of high-quality prospective randomized controlled trials (and thus with the need of cautious interpretation), cytoreductive radical prostatectomy in combination with hormonal therapy may prolong progression free survival, cancer specific survival, and overall survival. Compared to hormonal treatment alone, surgery leads to significant reduction of local complications associated with disease progression. Functional outcomes and incidence of complications do not differ from radical prostatectomy performed for high-risk localized prostate cancer.
