Description The authors describe the case of a 48-year-old female patient who developed from acute erysipel on the right lower extremity a phlegmonous inflammation affecting deep subcutaneous tissues incl. muscle fasciae-fasciitis necroticans.In the development of the disease some risk factors were involved such as venous and lymphatic stasis associated with venous chronic insufficiency, diabetes mellitus and previous injury of the knee joint treated surgically.The clinical picture involved oedematous reddening of the leg with tongue-shaped spread to the thigh, vesicles, bullae, necroses and ulcerations. The dramatic local finding was associated with high temperatures aned general alteration of the organism. from the etiological aspect in the condition, no doubt, a beta- haemolytic Streptococcus participated as confirmed by the high ASLO value (2000u.). In addition to the beta-haemolytic Streptococcus on cultivation the following were detected: Staphylococcus pyogenes,Pseudomonas aeruginosa, Proteus vulgaris, Escherichia coli and as to anaerobic microorganisms Bacteroides sp. Treatment was successful after large doses of broad spectrum antibiotics, necrectomy, local therapy respecting the stagewise healing of ulcerations and external compression.
