Description Acute stroke is a major cause of disability and other adverse long-term sequels in industrialized countries. There is strong evidence that outcomes after stroke can be improved with appropriate treatment. Patient with acute stroke should be evaluated and treated immediately in a stroke care unit. Intravenous administration of rtPA (0,9 mg.kg-1; maximum 90 mg) is strongly recommended for treatment of carefully selected patients, who can receive the medication within 3 hours of onset of stroke. An elevated blood pressure should be lowered cautiously. Urgent administration of anticoagulants (except patients with high-risk cardiac diseases) and neuroprotective medication is not recommended. Effective preventive measures include treating hypertension, dyslipidaemia and diabetes, antiplatelet agents, anticoagulants, smoking cessation, carotid endarterectomy and endovascular procedures in patients with, or at risk for, high-grade carotid-artery stenose. Considerable research in stroke has been done in the last decade and much additional work needs to be done.
