Description Opremcak as a method first presented radial optic neurotomy in 2001 for treatment patients with central retinal vein occlusion (CVO). Nerve fibers and arterial supply go through a very narrow scleral channel. This sector operates under Opremcak like compartment, which potentially narrows the lumen of any attack of the central retinal vein. A radial incision of scleral edge of the optic nerve is released from his grip and opens the compartment. Decompression of central retinal vein then accelerates and improves blood flow. The authors represent published cases report 2 patients with occlusion of the vena centralis retinae, which was conducted by RNO via pars plana vitrectomy (PPV). RNO is a method of treatment reserved for the unfavorable prognosis of ischemic CVO. Very important role in the pathophysiology of RNO plays not only release scleral tunnel through which the optic nerve (compartment cancellation effect), but according to the latest messages in literature creation of chorioretinal collateral in the optic neurotomy. These are then a crucial part in the creation of collateral venous circulation in the exit of the optic nerve.
