Description Evidence-based guidelines for the management of patients with Lyme borreliosis have been prepared by the experts of the Society of Infectious Diseases and are supported by appropriate medical branches of the Czech Medical Society of J. E. Purkyniae. LB is the most common tick-borne infection in the Czech Republic. Its course is mostly asymptomatic. Involvements of the skin, nervous system, joints and heart are the predominant clinical manifestations. Classification of the disease and clinical forms of early and late disseminated stage are discussed in detail. Laboratory diagnosis is based on two-tiered testing of antibodies and in neuroborreliosis, on simultaneous CSF examination including intrathecal synthesis, in Lyme arthritis on detection of antibodies in synovial fluid. Guidelines provide recommendations on antimicrobial treatment with regard to the clinical manifestations and the availability of effective antibiotics in CR. The duration of recommended therapy should not exceed 28 days.
