Description European guidelines for diagnosis and treatment of hypertension were published in 2007, followed by British guidelines in 2008, and so called REAPPRAISEL in 2009. The British NICE guidelines were published in 2011 and the Czech guidelines in 2012. The guidelines are similar of course, but some differences do exit. Czech Republic follows the European guidelines in the classification of hypertension (mild, moderate and severe) and 3 grades in prehypertension (optimal, normal, high normal). Combination treatment is the preferred choice if pharmacological treatment with the stress on concomitant diseases. Normalization of the systolic blood pressure is the main goal. Renin angiotensin aldosteron blockers – ACE inhibitors and AII antagonist are so called universal drugs and diuretics and calcium channel blockers are recommended to the combination. Indapamid is the preferred diuretics, followed by chlorthalidon. Hydrochlorothiazide is critiyed. Fixed combinations are preferred to increase the adherence of the patient.
