| Description | The term non-radiographic axial spondyloarthritis (nr-AxSpA) refers to a stage of axial spondyloarthritis without radiographic sacroiliitis in the presence of typical clinical manifestations of the disease. For some patients active sacroiliitis is detectable by magnetic resonance imaging, in some patients the diagnosis could be based on the composite of clinical symptoms and signs and presence of HLA-B27 antigen. Clinical and laboratory characteristics of nr-AxSpA are comparable with ankylosing spondylitis (AS). Also current treatment strategies for nr-AxSpA are the same as for AS. They include non-steroidal anti-inflammatory drugs as first-line therapy and inhibitors of tumor necrosis factor-alpha, effective in treatment of axial and peripheral symptoms of the disease including a positive effect on extraskeletal manifestations. Recently the use of several anti-TNF drugs was approved in patients with active nr-axSpA, allowing to intervene early in the disease. |
